GYNCOLOGICAL STDs
  1. Chlamydia
  2. Gonorrhea
  3. Condylomata Acuminata
  4. Molluscum Contangiosum
  5. Herpes Simplex
  6. Syphilis
  7. Chancroid
  8. Granuloma Inguinale
  9. Lymphogranuloma Venereum

Chlamydia trachomatis most common STD often associated with N. gonorrhea 

 

Risk factors

  1. < 25 years old
  2. history of previous STD
  3. new partner in last 3 months
  4. multiple partners
  5. not using barrier contraception
  6. contact with infected person

 

Symptoms 

  1. asymptomatic
  2. muco-purulent endocervical discharge image
  3. urethral syndrome
  4. dysuria, frequency, pyuria, no bacteria
  5. pelvic pain
  6. post-coital bleeding

 

Complications

  1. acute salpingitis, PID
  2. infertility - tubal obstruction from low grade salpingitis
  3. perinatal infection - conjunctivitis, pneumonia
  4. ectopic pregnancy
  5. Fitz-Hugh Curtis syndrome (liver capsule infection)
  6. arthritis, conjunctivitis, urethritis (Reiter syndrome - male predominance)

 

Diagnosis

  1. cervical culture or monoclonal antibody
  2. obligate intracellular parasite - require tissue culture for diagnosis

 

Treatment

  1. doxycycline 100 mg bid for 7 days
  2. or azithromycin 1 g orally in a single dose
  3. erythromycin 500 mg qid for 7 days if pregnant
  4. treat partners
  5. reportable disease

 

Screening

  1. high risk groups
  2. during pregnancy

Neisseria gonorrhea

 

Symptoms and risk factors as with Chlamydia

 

Diagnosis

  1. Gram stain shows gram-negative intracellular diplococci
  2. cervical and rectal and throat culture

 

Treatment

  1. single dose of ceftriaxone 250 mg IM or cefixime 800 mg PO or ciprofloxacin 500 mg PO
  2. plus doxycycline 100 mg bid for 10 days to treat for concomitant chlamydial infection
  3. erythromycin 500 mg qid for 7 days if pregnant
  4. treat partner(s)
  5. reportable disease
